Targeted Case Management (TCM)

When a child is struggling with emotional, behavioral, or mental health challenges, families often find themselves trying to navigate multiple systems at once. Appointments, school meetings, referrals, community resources, insurance requirements, and treatment recommendations can quickly become overwhelming.

At Bluegrass Behavioral Services for Children (BBSC), our Targeted Case Management (TCM) program helps families connect the pieces. We serve as a guide, advocate, and coordinator, helping children and families access the services and supports they need to improve functioning, increase stability, and achieve their goals.

Targeted Case Management is not therapy or direct treatment. Instead, it is a medically necessary service that focuses on identifying needs, developing a plan, coordinating care, connecting families with resources, monitoring progress, and helping ensure that services are working together effectively.

What Is Targeted Case Management?

Targeted Case Management is a service designed to help eligible children with serious emotional, behavioral, or mental health needs gain access to medical, behavioral health, educational, social, and community resources.

Many families know their child needs help but are unsure where to begin. Others have already connected with services but struggle to coordinate care between providers, schools, and community agencies. TCM helps bridge these gaps by ensuring children and families receive the support they need in a coordinated and organized manner.

Our case managers work closely with families, treatment providers, schools, community agencies, and other involved supports to create a comprehensive, person-centered approach that addresses the unique needs of each child.

How BBSC's TCM Program Helps Families

Assessment and Identification of Needs

Every child and family has unique strengths, challenges, and goals. The TCM process begins with identifying areas where support may be needed, including:

  • Mental health and behavioral health services

  • School and educational supports

  • Community-based services

  • Family support resources

  • Medical and healthcare needs

  • Social and recreational opportunities

  • Housing, transportation, or other community resources when applicable

Our goal is to develop a clear understanding of the child's needs while recognizing and building upon family strengths.

Person-Centered Service Planning

After identifying needs, the case manager collaborates with the child, family, and other supports to develop an individualized service plan.

This plan outlines:

  • The child's strengths and needs

  • Goals and desired outcomes

  • Resources and services needed

  • Steps required to obtain services

  • Strategies for overcoming barriers to care

The service plan serves as a roadmap that guides coordination efforts and helps ensure services remain focused on meaningful outcomes.

Care Coordination

Children with significant behavioral health needs often receive services from multiple systems simultaneously. Our case managers help facilitate communication and coordination among:

  • Behavioral health providers

  • Medical providers

  • Schools and educational personnel

  • Community agencies

  • Child welfare agencies when applicable

  • Other service providers involved in the child's care

Effective communication helps reduce duplication of services, identify gaps in care, and improve overall outcomes.

Resource Linkage and Referral

Many families are unaware of available resources or uncertain how to access them. BBSC case managers help connect families with services and supports that may benefit their child and family.

Examples may include:

  • Behavioral health treatment services

  • Educational supports and accommodations

  • Community-based programs

  • Family support resources

  • Recreational and social opportunities

  • Medical and specialty healthcare services

  • Crisis resources when needed

Advocacy and Support

Navigating systems can be challenging, especially when families are managing a child's complex emotional or behavioral needs.

Our case managers advocate alongside families by helping them understand available options, prepare for meetings, access services, and overcome barriers that may interfere with care.

We believe families are the experts on their children, and our role is to support and empower them throughout the process.

Monitoring and Follow-Up

Needs change over time. As children grow and circumstances evolve, service plans and supports may need adjustment.

Case managers regularly monitor progress toward goals, review service effectiveness, identify new needs, and help ensure children remain connected to appropriate resources and supports.

Who May Benefit from TCM?

 

Targeted Case Management may be appropriate for children who have significant emotional, behavioral, or mental health challenges and who require assistance accessing and coordinating multiple services and supports.

Children may benefit from TCM when they:

  • Have difficulty functioning at home, school, or in the community

  • Require services from multiple providers or agencies

  • Are at risk for more restrictive levels of care

  • Need assistance accessing community resources

  • Experience barriers to obtaining needed services

  • Require ongoing care coordination and advocacy

Eligibility is determined according to Kentucky Medicaid requirements and medical necessity criteria.
